1. Identity & Household (Required for All)
Proof of Identity: Driver’s license, state ID, or passport.
Social Security Numbers: Required for all household members applying.
Proof of Residency: Lease agreement, rent receipt, or a utility bill in the applicant's name.
2. Income & Employment (Crucial for the 80-Hour Rule)
Earned Income: Last 4 weeks of consecutive pay stubs.
Unearned Income: Award letters for Social Security, SSI, Veterans Benefits, or Child Support.
Work Verification: If working, a Loss of Income/Employment Verification form (Form CF-ES 2620) if hours have recently changed.
3. The "New 2026" Mandatory Expenses (No longer self-reported)
Shelter Costs: Current mortgage statement, property tax bill, or homeowner’s insurance premium.
Utility Proof: Recent electric, water, or gas bill (to qualify for the Standard Utility Allowance).
Medical Expenses: If age 60+ or disabled, provide receipts for prescriptions, co-pays, or medical transport (only costs over $35/month).
4. Work Requirement Exemptions (If applicable)
Medical Exemption: A signed Medical Verification Form from a doctor stating the person is "unfit for work."
Caretaker Status: Proof of the age of the youngest child.
Pro-Tip for FL-16: If a resident cannot access a scanner, they can use the resource rooms at the Sarasota or Bradenton Career Centers to upload these documents for free.
Request Reimbursement: Ask your Career Coach for Food Stamp Reimbursements (FSRs), which provide funds to cover transportation (bus passes/gas) and childcare needed to attend training.
